Testosterone: We Test First
Low energy, low mood, poor sleep, less interest in sex and losing muscle are all things low testosterone can cause. They are also things that depression, thyroid problems, sleep apnoea and simply working too much can cause. The symptoms do not tell you which it is. A blood test does.
So we measure before we treat. That means testosterone, and alongside it PSA and a blood count, because those are the two things replacement therapy can move. If your level turns out to be normal, we say so and look for another explanation rather than treating you anyway.
What we measure before starting
- TestosteroneLevels vary through the day, so morning is the usual time to draw
- PSAReplacement is not started without knowing this
- Blood countReplacement can raise red cell counts, so we need the starting point
- Liver and kidney, blood sugarCommon alternative explanations for the same symptoms
Before You Start Testosterone
These are the trade-offs. They are not reasons not to do it, but you should know them before you begin.
- It suppresses sperm productionIf you want children, now or later, say so before we start. This is the single most important thing on this page
- It can thicken the bloodRed cell counts can rise, which is why we recheck them
- It affects the prostateWe check PSA before starting and again while you are on it
- It is ongoing, not a courseStopping returns you to where you started, or lower for a while
- Acne and fluid retention are common early onUsually settle
We do not prescribe testosterone or anabolic steroids for building muscle. If that is what you are after, this is not the clinic for it. What is on this page is replacement for men whose own level is low and who have symptoms to match.
Erectile Dysfunction
It is often a signal, not just a symptom
Erections depend on blood vessels working properly, so difficulty can be the first sign of a circulation problem — high blood pressure, diabetes, high cholesterol. That is why we examine and take bloods rather than simply writing a prescription. Finding the reason is worth more than treating the symptom.
Treatment and its risks
The usual medications work by relaxing blood vessels. Common side effects are headache, facial flushing, a blocked nose and indigestion. They are usually mild.
They must never be combined with nitrate medication — the kind prescribed for angina, including nitroglycerin sprays and tablets. Together they can drop blood pressure dangerously. Tell us everything you take, including anything bought without a prescription and anything sold as a supplement for the same purpose.
We also see men who have bought these online. Counterfeits are common and what is in them is unknown. If you have been taking something you are unsure about, bring it.
Frequently Asked Questions
Is the consultation private?
Yes. What you tell us stays between you and the clinic. We do not contact your employer or anyone else.
Can I just get a prescription without tests?
No. For ED we examine and take bloods first, because difficulty with erections can be the first sign of a circulation problem worth finding. For testosterone we always measure your level first.
I want to have children. Can I still have testosterone?
Tell us before we start. Replacement suppresses sperm production, and that matters if children are on the horizon. There are other approaches, and it is a conversation worth having first, not after.
Do you prescribe steroids for training?
No. We do not prescribe anabolic steroids and we do not treat for bodybuilding. What is here is replacement for men with a genuinely low level and symptoms to match.
Do I need Japanese health insurance?
No. This is paid out of pocket either way.
Can you write a letter for a doctor at home?
Yes. We write results and treatment summaries in English, including reference ranges, so someone can pick it up where we left off.
