CERTIFICATES

Certificates in English

We issue vaccination certificates and medical letters in English, and complete forms supplied by your school or employer.

  • Open 7 days, 9:00–19:00
  • 1 min from Asakusa Station
  • English certificates

Overview

Schools, employers and visa offices each want documents in their own shape. Sometimes free-form is fine; sometimes a doctor has to sign a specific sheet of paper.

We see international patients every day and write documents in English as a matter of course. Bring the form you were given and we will fill it in with you.

What we issue

  • Vaccination certificate in EnglishFor vaccines given here
  • Medical letter in EnglishIncluding your own required format
  • Translation of a vaccination recordFrom a Boshi Kenko Techo or records from your home country

How It Works

Book your visit

Use the form at the bottom of this page or call us. Telling us your destination and departure date in advance makes your visit faster.

Consultation

We go over your destination, your schedule and your vaccination history, then explain which vaccines you need, how many doses and what it costs.

Vaccination

We observe you in the clinic afterwards and explain how to spend the rest of the day, along with side effects that can occur and what to do about them.

Next dose & certificate

If you need more than one dose we book the next date on the spot. Tell us if you need a certificate.

What to bring

  • Photo ID
  • Your vaccination record, if you have one (a Boshi Kenko Techo, or records from your home country)
  • Any form your school or employer has asked us to complete
  • Your travel itinerary or departure date

Before You Book

Does health insurance cover this?

Travel vaccinations are not covered by Japanese health insurance and are paid out of pocket. We explain the cost during your consultation.

I am leaving soon. Is there still time?

We work with the time you have. Even for vaccines that need several doses, there is often a way to fit in the doses that count most before you go. If we cannot finish in time, we explain what to watch for at your destination and what to do if you fall ill there.

Can I be vaccinated on the day I come in?

Yes, for vaccines we have in stock. Some have to be ordered in, so please call ahead if you already know what you need.

I am worried about side effects. What can happen?

Pain, redness or swelling where you were injected, along with fever, are the most common. We explain what can happen and what to do about it before you are vaccinated, and we observe you in the clinic afterwards.
